As a community facility we are required to maintain a contact register for Covid-19 tracing purposes.
To make it easier for yourself, download the Safe WA app which will enable you to scan the QR code, located at our front door, whenever you enter the building.
If you are not able to download the app then you will need to fill out the form with your contact details, located on our front desk.
